With traffic lights it's the yellow and red that are harder to tell apart because the green is practically white. I can tell the difference by the light's position on the traffic signal though (yellow is in the middle).

Only time I screwed up was when I was driving at night one time, and there was a solo flashing red light at an intersection. The red light was unusually bright. Instead of stopping at it, I slowly drove through it w/o stopping because I thought it was flashing yellow. Flashing red is like a stop sign, but flashing yellow simply means slow down. That was when I was 16 though.
