Default Books on Behavior Modification?

I've been looking for self-help books on behavior modification, but there doesn't seem to be any, only huge text books for psychology students.

Can anyone recommend a book on do-it-yourself operant conditioning.....using rewards, score cards, etc for forming new habits?
