Default Paper-based GTD

Does anyone have a good GTD system that's generally paper-based (with the exception of email, Excel/Word/pdf files, and other basic computer stuff)?

My main problem in maintaining my system is updating my next action lists with new next actions. For example, if I make a certain phone call for Project A, there's nothing that automatically lets me know that the next next action for Project A is X. I'd also like some creative suggestions for what you write your lists on, what containers you use for project material, how you store the material, etc. Do you have a cool way of sub-organizing large projects? Dumping 50 pieces of paper into a folder lacks style.

I'm interested in hearing anything you have to say about paper-based GTD systems. Thanks - Sky.

My GTD system is based around my Planner Pad. Everything goes in there.

I'd go into further detail, but that's pretty much the only unique thing about it. I work out of my car, so my briefcase is my inbox.

I am not sure if you already visited the following site. It has very good paper based planners.
