Is this currently available? Is it a supplement or a pharmaceutical?

Sublingual (melts under your tongue) methylcobalamin has been available as an inexpensive supplement at standard grocery stores for years in the US, it's just that most multivitamins still use the even cheaper cyanocobalamin. Go wherever you would usually buy vitamins and you'll likely find it.

It's in meat, and most vegetarians don't get enough of it.

Actually someone who tests for B-12 found that about 40% of vegetarians have it and 40% of meat-eaters have it. Many have problems assimilating it. Fasting on only pure water has cured vitamin B-12 deficiency.
