The depression may be the cause. If you are depressed, you will not feel good doing homework. Believe me, you need the right motivation to do your homework.

So, either it is purely depression causing your problem or you just need a passion to make you motivated. I find that doing things I love will DEFINITELY improve my mood to do homeworks.

I don't know about your condition except what you have explained here. You sound very informed and aware and describe a situation that sounds like there is extra physical stress. You also seem aware that you are getting away with not working as hard as you can.

I bet, that if there was a assignment or topic you were interested in, you would take the time to study it. Maybe you could try to be more creative in your home schooling, there are fun lesson plans that may be more interesting than what you are learning now.

All teens go through a period of losing friends and isolation. Successful young adults will find ways to make new connections. Not attending a school or being able to get out of the house is not a good excuse today for not connecting. I bet you have a phone with text and you have a computer with internet, so you can reach out and make and keep friends.

I don't know if it's directly related to your health, but it sounds like something any normal teenager could be going through.

It could be because you're relying on things like your parents' anger to control your behavior. You should reevaluate the value of accomplishing something that requires such negative reinforcement. You don't want to live a life relying on negativity from yourself and others to be the force that drives you forward. If your means are something like that, then the end isn't worth it, since the end is always a function of the means. Unhappy means will lead to an unhappy end and an unhappy life.
