Default Change

I'm afraid that if I change, I will fall back into my old patterns again. I'm afraid of reaching a new level, and then falling back, so then I become afraid of trying to make a change because I feel like it won't last because I have fallen backwards so many times.

Can someone help me get past this? I am at a point in my life where I need to make a serious change now but I am having difficulty because I don't think I can break through and live my life in a new way.
