Break it up into chunks and focus on having each part happen. If you're planning on being a doctor, for example, focus on completing a certain required class, then on all your premed reqs, on taking the MCAT, on applying to various schools, and so on. One of my friends seems to be doing quite well at this right now, having taken all his his premed reqs before his junior year and now he's about to take the MCAT, thus why that example came to mind.
