Manifestation would seem to imply, a physical output, where as creation would seem to imply physical and non physical.

Exactly, where is it created, does a choice, a thought instantly manifest and then you don't see it until you fully accept it or further choose to render it in awareness and physical reality.

At that point, I would have thought it is just an idea in my mind.

That's the ultimate question, how does a non physical thing like choice and thought manifest, create a physical output. If I choose to have $5000, how does that output into the physical reality and why doesn't it happen if it doesn't appear.

Obviously no one can say for sure exactly how creation works when a non physical thing like thought creates a physical output, but I think the answer could lie in reversal. Most people seem to think that their physicality is creative, that things are created through work or physical intent, but if everything that has ever been created started out as thought or more specifically choice, then choice must be the creative source/force.

So it would seem that physicality has very little or no creative power. If I want $5000 and I choose to have it and validate it with affirming thought and then somehow it manifests, then it was the thought, the non physical thing that created it and all I did (my physicality) was aid in it being rendered into reality, but it was already there, it just needed to be uncovered, fully realised.

Could it be that everything we require in existance is already here right now, but we're denying it to maintain a believable reality? Could it be that I already have $5 million dollars in my account, I'm just not seeing it? Same goes for love and health.

When we place our perception with physicality, we are limited to that physicality. We can't create there, it's a place of output only, it's the rendered, never the rendering. So to be more creative we must believe, we must know 100% that everything is created outisde of physicality and possibly is already fully rendered and available.

How fast can we render stuff into our physical reality? I'm guessing it's done instantly and we are simple denying it's already there to maintain some kind of acceptable reality. If you can't see the million dollars in your account, it's because you can't see it. You are actually seeing, making aware to yourself a choice/thought of the past.

This thread is about the past infecting the present, so you don't have the million dollars in your account because you look to the past and remember it not being there and then bring that choice of not having it into the now and the continuation of the past.

I droned on and not sure wether I answered your question, but creative power has the ability to render in physical reality anything, we just have to accept that it works constantly and consistantly and kill off the past and rewrite the reality as we desire. It doesn't seem to be that hard once we accept that we are creating what we're already getting, including all the stuff we say we don't want.
