I know quite a bit about screen readers for web-pages, which are primarily popular because semantic HTML markup (XHTML) nowadays makes it much easier for the software to read it off element by element (rather than an extremely long run-on sentence).

I am not at all familiar with screen-readers for desktop applications, I would venture to guess that whether you had a DOS command-line open or a GUI window open, either would be read equally by the software...
