The difference, for me, is that I don't initiate or visualise spiritual things. They come to me, and they're not actually in my own head. They're experienced in a space sort of... outside of my head, but not part of the material plane. It's a bit like having an earphone in one side that lets me hear (and otherwise experience) spiritual phenomena while still being aware of the material world around me.

Pretty sure I'm not just imagining it, because there have been plenty of experiences that other people also had, independently of me (earthbound spirits, for example, and negative imprints, that sort of thing).

A thing doesn't have to be literal to be true, and, as Joan said, how else would God (or whomever) speak to me? (I guess he could send a televangelist, but I'm not too likely to listen, are you? )
