I start with paper, and then write down what I'm trying to achieve with the site and what feel I want it to have. After that, I'll sketch a tonne of designs for the front page, noting where all the important elements should go. Once I'm done on paper, I'll head to Notepad and start typing until I've got something that looks decent.
Then I'll look at a CSS gallery and be upset that my site sucks
