my experience:
working on BOTH aspects simultaneously (...of course, since everything is simultaneous )

1- fostering positive anchor: recreate your Reality (mainly on the "Ethereal" Plane desire/vibrational shift etc)
2- breaking negative anchors: FACE YOUR FEARS HEAD ON on the "physical" Plane

the higher your level of Consciousness, the less you need direct "physical" action
the ratio Ethereal/Physical action is dependent on your Awareness
