You're doing well, I have several violations just while sitting and studying. Try doing something called mental monitoring, in which you are mainly focusing on reality (ie the book in front of you) and you periodically "watch" your mind to make sure it's on track. Eventually, your watchman will occur pretty automatically and he won't be so intrusive. Whenever you're off track, make a mark on a piece of paper beside you and get back to work. You can make a game of seeing how few marks you get.
Of course, there is also having the right environment for you to focus. Although you must be conscious in all of your endeavours whether they're social or public, you have great control over your study environment at home. You can eliminate all distractions and sounds that send you off thinking about other things.
Also, I recommend some kind of list-making such as Getting Things Done if you worry about tasks you'll have to do in the future. Get them out of your memory so they stop popping up, and just write them onto a pad immediately and go back to your task.
