One way it can do that is if you have tried to find your friends online by logging into your Hotmail/yahoo/gmail account through Facebook. It collects all the email ids in your mails and tries to match them up with existing Facebook accounts. So most probably, your ex did that and it showed you as a possible friend. (Never do that yourself, btw. It is very risky.)
