Do you think your feelings about not doing well are affecting they way you feel about the course. If you were getting good grades would you want to be there?
If that's the case, then perhaps you need some extra tuition to get you back up to speed and maybe some exam coping techniques and essay writing help to help you get the good marks again and help you start enjoying your course again.
If good grades wouldn't make a difference, then don't jump ship before you have decided what it is you really want to do and putting a plan into place. A vague sense of 'helping people' won't be enough. You'll just drift for a while which will just create more worry for you. Be definite. Decide where you want to go and what you need to do to get there.
I'm picking up on your worry from your post, so think you would do well to talk it over with someone from your college, either a tutor or careers advisor or other pastoral care who will be able to discuss with you what some options are and help you get a plan together.
