Ok, You are thinking way too much into this. And that is what is killing you.
SHe is just a girl. Nothing special. Maybe she will be maybe she wont', but you will NEVER find out if you keep thinking about this and not do something about it.
Whatever happened in your past, its just that, it just something happened in your past. No reason to bring this up, because it is pulling you down right now. You are making all sorts of excuses of why you can't have her.
Here is what you do:
Next time you go to Applebees, go without your friends and go before rush hour, when servers aren't doing much. It is usually around 2-5pm.
Walk up to her and say:
"Hi, my name is so and so. I find you very cute. If I don't do this I would be kicking myself later. Let me get your phone number"
At this point pull out your phone and give it to her for her to put in the phone number.
Smile and say nothing else.
