Check out the "Deep Sleep: 101" book/CD combo. That has helped me some when I had sleeping problems.
Also consider going to a hypnotherapist (this is different than a hypnotist). I recommend working with somebody who doesn't actually put you in an unconscious trance, but rather, puts you in a conscious trance (i.e. you're deeply relaxed and have greater access to the unconscious, but your conscious is still aware and involved). This approach also helped me discover some things when I had some sleeping problems.
