I've never come across a really credible definition of either term, the way I interpret self confidence versus self esteem - and the way I've treated them in my course - is that self confidence is the outward projection of confidence while self esteem is required to ensure your confidence is in line with your inner beliefs, instincts, and inner voice.
It's typical that both would go together, however there is always the possibility that you can project a false sense of self confidence where your self esteem is taking a big hit - this is especially common in certain phases of addiction where the addict puts his or her entire being into convincing those around them they are alright inside by projecting confidence - but they are really dying inside of low self esteem.
