Yea, I understand what you are saying!
What I do is at the end of each week I ask myself what I would like to achive and I list out everything and from there I work out what is next. I jot down what I need to focus on for the next week and then toss it away. Leaving only what is important and what needs to be done in the now.
At least that is what i'm going to try to start doing!
