A little bit of mental reprogramming really helps. Replacing positive associations (like "ooh yum a treat") with negative ones (slugs poo mud etc)
I stopped having a relationship with chocolate by sticking some at eye level in the kitchen cupboard and saying "chocolate is mud chocolate is mud" every time I saw it. It only took a few days before I could not stand the thought of eating it.
