Both, actually.
Of course we should nuruture our spiritual ears. That's the basis of just about every established religion or medatative philosphy mankind has ever created. But for them to nurture our ability to hear them we first have to accept the fact that they can be heard and we need to acknowledge and understand that they have always been talking and communicating with us.

Much of what we perceive as "our inner thoughts" are actually our guides.
Have you ever done something you knew wasn't right and had that little voice in you "head" chastise you for it? We've created this concept of "conscience" as an internal monitor. Well, I guess in a way it IS internal, but in another way it's just as "external" in that those types of things are ususally our guides correcting, guiding, teaching us.
We've never NOT had our guides talking to us. Even before we were born, our guides were there preparing us for our physical existance, giving us pep talks, etc. They didn't stop once our physical bodies were born. So it's natural for us as vain, self-centered beings to think that "it's all about us" and discount their voices as ours.
Once we accept the fact that our Guides are present and communicating with us, we begin to recognize all the ways in which they do so. Those weird little "coincidences" that seem to pop up out of nowhere take on a whole new dimension. Thinking about someone whom you haven't seen in a decade and then stumbling into them at the gas station becomes more than just happenstance.
The whole world takes on a new level of meaning.
At least that was my experience in coming to live with my Guides. It may be different for others.
