I had the same problem as you, looking for 'samples' and forms to fill in. I realized that the exact form isn't so necessary as the actual plan, and since then have just been doing it along other's suggestions but whatever way works for me, if you know what i mean. There is no 'perfect way,' there is only getting it done.
