Could be anaphylactic allergy to something he is ingesting, drinking or being exposed to. He should be allergy tested at once and begin keeping a food diary.
It could also be a manifestation of anxiety but he definitely needs the allergy tests first. They can be deadly if not addressed.
With these symptoms, he should carry with him an Epi-Pen which is a way to dose yourself with epinephrine for breathing emergencies. Benadryl (diphenhydramine) capsules too. He is in a totally new country and foods, flowers, plants, insects and possible reactions to a parasite can all cause these reactions.
If they continue, he should travel home at once.
