One to the recent books that I read, which is helping me bridge the gap between "knowing" and "doing" is "KNOW CAN DO" by Ken Blanchard, Paul Meyer and Dick Ruhe.

Its a great book that helps to address the reason why we don't put our knowledge into action. He mentions the 3 reasons why we don't do what we intend to do:

1. Information Overload
2. Negative Filtering
3. Lack of Follow-up

Its a great book that I would highly recommend.
