Have you read or heard of "Getting Things Done" (GTD) by David Allen? Part of his system is to get those those thoughts out of your head and into a trusted system. The GTD system includes writing lists, which you've noted that it does a great job of becoming more focused. I personally like what they call the weekly review where I review all those list on a weekly basis (although for me its bi-monthly
