This book has some really good meditation exercises:

Amazon.com: Full Catastrophe Living: Using the Wisdom of Your Body and Mind to Face Stress, Pain, and Illness: Books: Jon Kabat-Zinn

It's kind of long, but that's mainly because it discusses the experimental evidence for the health benefits of meditation. You can just skip ahead to the exercises in the book if you want.
