Default Visualizing vs fantasizing

I'm wondering about the difference between visualizing what we desire and fantasizing about it. Visualizing would have an effect on reality while fantasizing woudn't: but I'm trying to get a clearer definition than that. How do you know which one you're doing?

What we're told is that we need to use our imagination to feel the event as if it were happening in the present. Does this always have an effect on reality? Or can our subconscious disregard our awesome good feelings as mere fantasy, even if we're fully feeling them in the present?

After all, feeling good during masturbation doesn't attract a similar reality.
